How to Pass a DOT Physical

8 min

49 CFR §391.41(b)

Nothing in the regulation says "pass" or "fail". The examiner decides whether you meet thirteen standards and for how long to certify you. Most drivers who lose time lose it to something they could have handled weeks earlier.

The five that cost cards

Across a working clinic the same handful of findings account for most short certifications and most deferrals. None of them are exotic.

FindingUsual outcomeLead time to fix
Raised blood pressureShorter certificate, or a recheck3-8 weeks
Untreated sleep apnoea signsDeferral pending a study4-10 weeks
Uncontrolled blood sugarDeferral or the §391.46 pathway6-12 weeks
Vision below the standardNew correction, or the alternative standard1-3 weeks
Missing specialist letterDeferral until it arrives2-6 weeks

Notice the last row. A deferral for a missing letter is not a medical problem at all — it is an appointment problem, and it strands a driver for weeks over paperwork that existed the whole time.

What you can change in three weeks

Book the exam three or four weeks out rather than the week your card expires, and the window turns into a real one. Blood pressure responds to sleep, salt and taking the medication you were already prescribed. Vision responds to an up-to-date prescription — the standard is 20/40 Snellen, with or without correction, and correction counts, so an old pair of glasses is a solvable problem, not a disqualifying one.

If you take a medication that needs a treating-clinician letter, ask for it the day you book. Bring the original. Examiners will not certify on a photograph of a letter they cannot verify.

What to bring on the day

  • 01Glasses or contact lenses, and hearing aids if you use them — both are allowed by the standard.
  • 02A written list of every medication with doses, including the ones you take occasionally.
  • 03Letters from treating clinicians for any monitored condition, on letterhead, signed and dated.
  • 04Your CPAP compliance download if you use one — 30 days of data settles the question in a minute.
  • 05Your last certificate, so the examiner can see the previous term and why it was set.

Skip the energy drink on the way in. Caffeine and nicotine in the hour before the reading are the cheapest way to turn a normal blood pressure into a short card.

If you get a short card

A three-month or one-year certificate keeps you working. Take it, put the recheck in the calendar the same day, and treat the interval as what it is: the examiner telling you exactly what to fix and giving you time to do it. A driver who arrives at the recheck with three months of readings usually walks out with the full 24 months.

Out-of-service risk. Driving after the certificate expires is not a warning-letter offence. An expired medical card at roadside places the driver out of service, and for a CDL holder the licence status downgrades until the new certification reaches the state.
