A driver must be examined and certified at least every 24 months, and more often on four named pathways. The certificate is the examiner's output; keeping it valid on the driving record is the driver's job.

Those are ceilings, not entitlements. An examiner may certify for any shorter period they judge appropriate, and a three-month card for blood-pressure monitoring is completely ordinary.

Self-certification and the state

A CDL holder tells the state licensing authority which of four categories they operate in — interstate non-excepted, interstate excepted, intrastate non-excepted, intrastate excepted. Only the non-excepted interstate category requires the federal medical certificate to be on file with the state.

That self-certification is why the same driver can hold a valid paper card and still have a downgraded licence: the category on file says one thing and the certification status on the record says another. If you moved from intrastate to interstate work, the self-certification is the step everyone forgets.

For a CDL holder the carrier satisfies its file requirement by pulling the CDLIS motor vehicle record when it carries the certification status — the paper card is the driver's copy, not the carrier's proof.

Renewing without a gap

  1. 01Book the exam three to four weeks before the expiry date, not the week of.
  2. 02Submit the new certificate to the state the same week — most states accept an upload, and none of them chase you.
  3. 03Check the driving record a fortnight later and confirm the status actually changed.
  4. 04Give the carrier a copy for the qualification file, and keep the original in the truck.

Step three is the one that saves careers. Uploads fail, clinics transpose numbers, and the driver is the only person who finds out before an officer does.

When it lapses

The day after expiry the driver is not medically qualified. The carrier may not let them drive, and letting them anyway is a violation on the carrier's record as well as the driver's. For a CDL holder the licence itself downgrades to non-commercial status once the state processes the missing certification, and getting it back means a new exam plus the state's own reinstatement process.

A downgrade is administrative, not disciplinary — but it takes days to unwind, and the truck does not move meanwhile. Track expiry dates in the same place you track the annual inspection and the registration.
